Author will present the use of communication and assessment tools for common pain conditions (i.e. fibromyalgia, migraines) and specific populations (i.e. adolescents). Practical tools to facilitate communication and maximize the use of clinic time will be featured. In addition, tools to facilitate tracking progress and assisting the patient with preparing for follow-up visits will be discussed. American Chronic Pain Association (ACPA) supported facilitators offer a community resource that can be leveraged by the healthcare professional in providing patient centered care. They will also focus on the self-management tools needed for a patient with pain to become an active participant in their health care.
